How to Request Accommodations

To request reasonable accommodations and support services through the Office of Educational Support and Disability Services (ESDS), please complete these five simple steps: 

The Number One

Complete the Disability Disclosure and Reasonable Accommodations form.

This form will need to be submitted every semester in which you require accommodations. Accommodations may change from semester to semester and will not be implemented retroactively.

Access to the form requires you to log in using your UMID and password. If you need assistance completing the form or a paper copy, please contact Disability Services

All accommodations are determined on a case-by-case basis, according to the needs of the individual and the individual’s specific program of study for a particular semester.

The Number Two

‌Submit appropriate documentation confirming your disability.

Documentation should be current and will be evaluated on a case-by-case basis.
 

Documentation may be sent by email or through the Disability Disclosure and Reasonable Accommodations form.

The Number Three

‌Meet with Us

Contact ESDS staff to schedule an initial appointment to discuss barriers to access, accommodations, and services that you may need.
 
Due to working remotely, most appointments will be conducted virtually via Webex, Zoom, Skype, or telephone until further notice. If you are interested in an in-person meeting, please specify. 


The Number Four

‌Connect with Your Instructors

After you receive the email notice confirming your approved accommodations, log into your account and download your Approved Accommodations Letter. 

Present your instructors* with your Approved Accommodations Letter to coordinate accommodations. It's good practice to also have a confidential discussion with your instructor about your accommodations shortly after giving them the letter.  

You aren't required to disclose medical information to your instructors. If you are asked questions that make you uncomfortable, please notify us.

*Due to unidentified grading, students from the School of Law will work first with the Director of Student Affairs regarding the coordination of accommodations. Students from the School of Law are expected to work directly with faculty to discuss reasonable deadlines on assignment accommodations.


The Number Five

Reflect on Your Accommodations and Revisit with Us

Pay attention to what's working, what's not working, and what kind of barriers you're still experiencing throughout the semester. Your accommodations aren't set in stone.

We want to hear about what's been working, what barriers still exist, and what you think could help. Contact us to talk anytime.  In addition, an evaluation will be sent at the end of each academic year to help us evaluate ESDS office services and programming.

Report a problem right away if you encounter any serious barriers to access at UMB.
